Following our successful launch in September 2016 of the Yorkshire and Humber Community of Practice, we come together again in February 2017 to:
* Discuss a case study to develop practical actions that improve services for people with complex needs
* Further explore the initial ideas developed at the 1st meeting
* Allocate funding to assist with the actions / ideas discussed on the day
* Discuss training opportunities
* Discuss how best to develop an online community
